Continuous Upgrade of Surgical Techniques in Traditional Chinese Medicine: Single Hole Dual Media Spinal Endoscopy (DMSE) for Lumbar Spinal Canal Expansion Decompression and Nucleus Medullae Removal

2024-06-10

Recently, the surgical team led by Shang Hongming from Qingyun County Traditional Chinese Medicine Hospital successfully treated two patients with severe lumbar spinal stenosis using the latest domestic single small incision (1cm) dual medium spinal endoscopy technology (DMSE technology).

Case One

Mr. Zhou, 48 years old,
Lower back pain accompanied by pain in the right lower limb,
Intermittent limping for 10 meters,
The right lower limb straight leg elevation test was positive at 45 degrees,
When lying flat, if the posture is incorrect, it can cause severe pain in the right lower limb

Preoperative

Mr. Zhou's imaging data showed stenosis in the right side recess of L4/5, with visible prolapsed nucleus pulposus tissue shadows. When talking about surgery, his face changed. When he was told that we could use the most advanced minimally invasive spinal endoscopy technology to relieve his pain, the patient finally made up their mind. The surgical team successfully removed the prolapsed intervertebral disc from a 1-centimeter small hole using exquisite technology. The pain in bed eased the next day after surgery, and Mr. Zhou's face also showed a long lost smile;

Mr. Zhou's postoperative recovery is good

Case Two

Preoperative

Mr. Li, due to back pain accompanied by worsening left lower limb pain for one year and three days, came to the hospital. The patient had severe pain in the left lower limb and left buttocks, and was afraid to get out of bed. The left lower limb straight leg lift test was 15 degrees, and the pain was severe and unbearable. The left lower limb was numb, and there was a history of right lower limb pain and numbness. Imaging data showed disc herniation in L4/5 and L5/S1, calcification of the posterior longitudinal ligament in L5/S1, and spinal stenosis. The patient is in a serious condition, young in age, and requires high physical labor. After careful review of the film and selection of surgical plans by Director Hongming of Commerce, it was ultimately decided to perform a minimally invasive surgery using a posterior dual medium spinal endoscopy. During the surgery, three dislocated intervertebral discs were removed, and the calcified posterior longitudinal ligament was completely removed, achieving the purpose of unilateral and bilateral decompression. The patient's lower limb pain immediately relieved after surgery, and the joy of removing the pain appeared on Mr. Li's face.

Dr. Shang Hongming performs minimally invasive surgery with dual media spinal endoscopy for patients

Remove large pieces of intervertebral disc tissue during surgery

Compared to others
Under endoscopy (DMSE) can avoid the shortcomings of open surgery
Transform the traditional 10 centimeter large wound
Transformed into a minimally invasive hole of 1 centimeter
Achieve minimal trauma and fast surgical outcomes
The advantages of short recovery time and low treatment costs
The surgery was very successful
Intraoperative bleeding of only 10 milliliters
Pain symptoms disappeared in both patients after surgery
Urinary and fecal recovery function
Get out of bed and move around on your own 2 days after surgery

What is dual medium spinal endoscopy?

Director Shang Hongming introduced that dual medium spinal endoscopy technology is a new endoscopic technique in the industry, which is improved by combining traditional intervertebral disc endoscopy with intervertebral foramen mirror water medium. As needed, water and air media can be switched at any time to meet the actual surgical needs of clinical practice.

With the arrival of aging population, the proportion of middle-aged and elderly people in surgical outpatient services seeking medical advice due to spinal diseases is increasing. More elderly people not only have basic medical diseases: hypertension, diabetes, heart disease (including after coronary stent implantation, pacemaker implantation, etc.), but also have degenerative diseases such as scoliosis and spinal rotation. On the one hand, patients find it difficult to tolerate open surgery (extensive decompression, bone grafting, internal fixation), and on the other hand, patients have extensive spinal stenosis (including central spinal canal, lateral recess, and nerve root outlet). This has led many elderly people to ultimately give up surgical treatment and choose to endure pain and limited mobility, bringing a great burden to their families and society. Dual medium endoscopic spinal surgery has a large operating space, high decompression efficiency, minimal soft tissue damage, and more effectively reduces the occurrence of complications such as neurovascular injury.
